Constipation is prevalent in various groups such as infants, old people, pregnant women and office workers who are sitting for a long time. And the symptoms of them are similar, but the causes differ from age, habit and other factors. Therefore, the treatment principles are determined by their actual situation.  


What is constipation?    


Constipation can be seen as a symptom of other diseases or a unique illness such as chronic constipation.  As the intestines move slowly to remove the metabolites, the toxins accumulate and damage your body, causing the symptoms of constipation to turn into the disease. The effect of constipation on those affected is based on their symptoms. They always experience hard and dry stools, painful and fewer than three bowel movements per week. 

To date, it’s difficult to give a precise definition of constipation, because of the huge differences in bowel-movement caused by the types of food, lifestyle, environmental factor and mental status. For example, some people defecate once every few days without feeling uncomfortable, which is part of a normal bowel movement. 

Constipation can occur in a mild or a  severe degree, in short, or long duration.  Patients with severe and persistent constipation should go to the hospital and undergo tests to find out the exact cause. The immediate and efficient diagnosis helps to prevent or treat primary affections. However, do not abuse the laxative even you suffer from severe pain. 


Main symptoms of constipation:   

   Fewer bowel movement
   Dry faeces  
   Difficulty  with bowel movements  
   Lower abdominal  bloating and pain  
   Poor appetite  
 
•   Nausea  
   Blood in the hard stool


Complications: 

 •   Colon or anal disease 

 •   An extra-intestine disease like stroke  

 •   Hepatic encephalopathy, breast disease, and Alzheimer’s disease  

 •   CVD (cardiovascular diseases) like angina and myocardial infarction. In some clinical cases, the risk of CVD s has been increasing in recent years if constipated patient increases abdominal-pressure and overexertion during the evacuation.

Cause of constipation during pregnancy   


As the high-risk group of constipation, once pregnant women suffer from constipation, it may damage both them and the baby. Specialists explain that the digestive period of the food in the stomach and intestines will extend from the fourth month of pregnancy because the hormone level inside changes irregularly. For example, the increased secretion of progesterone will slow down enteric-coated peristalsis and impede normal excretion. In the middle and later stages of pregnancy, the enlarging uterus increases the pressure on the intestinal muscle, which leads to difficulties in evacuation.  This can be explained in more detail: 


 1. The increasing uterus begins to push the colon, so that the faeces are transmitted at a slow rate, resulting in abnormal bowel movements. 


 2. Hormonal changes: The increasing progestational hormone can reduce the tension of the muscles in the gastrointestinal tract (intestine from the end of the stomach to the anus), which leads to a loss of defecation. 


 3. The eating habits change. For instance, the loss of fibre contained in coarse grain can reduce the number of faeces as well as the urge to defecate that could otherwise stimulate the intestinal wall. 


 4. During pregnancy, insufficient exercise could affect the convolutions of the colon. 


 5. Some pregnant women take medications such as sedatives to relieve the unpleasant symptoms. However, this medication can cause side effects in bowel function, which is the critical factor of constipation in pregnancy. 

  

From the point of view of Eastern medicine, pregnant women could suffer fro m internal injuries in the spleen and stomach.  In addition, inadequate intake of water and fluid loss through sweating can damage the yin and result in a deficiency of Qi and blood in the body. Moreover, the impaired Qi may cause the colon to move slowly, and insufficient blood can’t nourish the colon, which leads to a dry bowel and difficulties in defecation. 

Treatment of constipation during pregnancy   


1. Home therapy  

   Improve the nutritional structure. Increasing the intake of crude fibre food can stimulate the intestinal movement.

 •  Form a  regular habit of defecation. For example, go to the toilet after breakfast. 

 •  Do appropriate exercises 

 •  Keep a healthy mind 

 •  Take the laxatives rationally under the guidance of professional doctors. 

 •  Drink more water or honey 

 •  Eat figs and prunes (green plum products) 

 •  Prune juice is a kind of natural juice that eliminates constipation. In general, the effect would occur 1 to 2 hours after drinking. Drinking a small cup of it every day helps to improve the gastrointestinal function. 


2. Selection criteria for possible medications  during pregnancy are as follows: 

 •  Natural and non-toxic drugs  with precise active ingredients 

 •  Do not induce diarrhoea and soft stool 

 •  Safe and reliable. There should be no side effects and no dependence on the drugs. 

 •  Reasonable price, suitable for family use 

 •  There is no stimulation of the uterus, no function of teratogenicity and mutation. 

 •  Do not cause diarrhoea in infants or other side effects 

 •  High safety and good tolerance 

 •  Provides fibre and improves intestinal output, helping to restore the regular habit of defecation. 


However, laxatives should be limited for pregnant women, because some of the stimulant laxatives, such as castor oil and senna, may contract the uterus and, in severe cases, even lead to miscarriage. In contrast, some of laxatives have an effect on the accumulation of water in the intestinal cavity, which can enlarge the intestine and stimulate bowel movements. These laxatives include lactulose, sorbitol and salt water. Moreover, softener and expansive agents are widely used by pregnant women.


3. Massage therapy 

Besides drug therapy, massage can alleviate constipation symptoms at home. Here are two recommended methods

   Lie on the bed and press on the stomach with the right hand or both hands, rotating clockwise rhythmically and fluently with light force, lasting  3 to 5 minutes.

 •  Rub the ‘Tian Shu’ acupuncture point: 

Lie flat on the bed,  placing the middle fingers on the acupuncture point of Tian Shu (about three-fingers wide or 6.7cm on the left/right side of the navel), that can promote the gastrointestinal function, rubbing clockwise for 1 minute.

 

 



Prevention of constipation   


1. Eat mixed grains and rhizome  as the staple food. Replace white rice with brown rice and  ground rice, mixed with oats or other grains. Otherwise, rhizome foods like sweet potato and potato  help you to ‘go’ to the bathroom.


2. Ensure the intake of fruits and vegetables every day so as to get enough vitamins, minerals and  fibre. You can eat some ‘chewing’ vegetables such as bamboo shoots, celery, carrots, cabbage, spinach, lettuce, lotus root, pumpkin, bean sprout and onion. And the nutritious fruits include pear, peach, jujube, mango, citrus, orange, grapefruit, papaya, tomato, strawberry, pineapple and banana. All the above foods can promote bowel movements. Especially, eating apples with peel and plums can promise you a smooth bowel movement every day.


3. The best way to prevent constipation is to drink more water: at least 1700  ml on a normal day and 2500 to 3000  ml in the summer. Moreover, it’s better to drink a (500  ml) cup of cold boiled water after getting up in the morning, which can stimulate  a bowel movement.


4. In order to maintain the normal  function of defecation, besides the balanced diet mentioned above, the regular diet should obey the following rules:

   Have three meals  at regular times and quantities

 •  Avoid too much alcohol and overeating 

 •  Stop midnight snacks 

These rules can help stomach and intestine reduce the burden and build a healthily digestive movement.


5. Form the habit of regular bowel movements

Go to the bathroom regularly every day,  to form a stable biological clock.


6. Relaxation exercises. When you feel tired, try gently beating the back with  your fist, which can relieve back pain and stimulate gastrointestinal movement. When taking a shower, massaging your belly with your hands  can also reach the preventive goal.
